3. Once you have logged in, scroll to the bottom and there will be a purple button option to ‘Download Printable Version’
4. A pop up will appear with ‘Your PDF Password’ (which is your date of birth) please click the green button ‘Download Printable Version’
5. A new tab will open in your browser prompting you to enter your password. After you have entered this you will see your PDF report which you then need to download/save. This will be Depending on which browser you are using.
6. Once downloaded please email your report along with your date of birth.

4. Once you have logged in click the menu button (the three lines in the top right) and scroll to the bottom and click ‘View Full Site’.
   
5. When you can see the full website at the bottom you will see ‘Download Printable Version’ click this.

7. A new window will open prompting you to enter your password. After you have entered this you will see your PDF report which you then need to download / save. This will be Depending on which phone you are using.

When you have the PDF you are able to call, email or cancel on-line anytime to avoid a charge should you not wish to continue as a member
